Patient Records

A patient record can contain a wide range of information to the patient and their care. Not all panels need to be filled out - only those relevant to the patient care. Patient Details Patient identifiable data (PID) is captured in the patient details panel. If your Team has selected not to capture PIDs, or if this is a solo record then the PID fields are removed History  These fields capture the incoming complaint and any past medical history.  Notes Notes are free text fields to add clinical notes. An unlimited number can be added.  Each note is time-stamped and shows who posted it so can be used for instance for senior clinicians to add consultation notes.  Primary Survey This is a standardised primary survey data set taken from our sister product HEMSbase but can be customised to suit your service. Frailty We use the Rockwood Clinical Frailty Scale.l. Observations  Charts include interventions and medications to give a full, time-stamped view of patient care. You can manually enter in observations. Once a few are entered it will automatically generate an observation chart.  Monitor Integrations IndieBase connects with all major monitor brands — Zoll, Tempus, Schiller, Lifepak — for seamless data capture via cloud APIs. That means that as long as you have connectivity you will be able to download relevant monitor data and automatically create patient observation charts. These observation charts will integrate the timing of any interventions or drugs.This will also overlay any drugs or interventions which have been administered over time. Cardiac Arrest Our default data fields match the Warwick Out-of-Hospital Cardiac Arrest Outcomes (OHCAO) data set. The panel can be hidden if desired, but the data fields cannot be edited to ensure consistency.  Blood Tests Blood tests can be manually entered into IndieBase. These values are automatically added onto the charts. Drugs & Interventions Clinicians should select from the prescribed list of drugs and interventions. These are configurable for the Team template by the Admins. Sedation This includes extra fields such as rationale, what drug regime, complications. Built in the standard sedation scales for before, during and after (RASS scale) Intubation Standard set of intubation data including indication, where, times and result. Individual intubation attempts are captured separately to facilitate more granular review. Drawing . A panel for more pictorial representations and annotations. Media You can upload documents or photos of the scene. IndieBase can use the device camera and put images straight into the patient record without saving them on the device first. Patient Signature Customers will be able to sign. This can be customised so that you clarify what exactly the patient is consenting to. You can have multiple consent fields (e.g. for refusal of care, or for willingness to receive a patient survey).  Checklists Each Team can create their own Checklists (any number). These can be as a standard procedural checklist or as part of an SOP, such as a check list of criteria that must be satisfied to issue a certain medication. Patient Report You can generate an ePRF which is a formatted, pdf version snapshot of the patient record. If you later make edits to the patient record, you will need to re-generate an ePRF to make sure it’s current.  These can be sent to NHS email, printed or shared from your device. IndieBase uses the secure NHS email protocol so can send ePRFs directly to hospitals or ambulance services.
